Required Physical Exam Information

Students entering Kindergarten, sixth grade, ninth grade, and students new from out of state are required to have a physical examination prior to the start of school.  Students in Kindergarten and students new from out of state must have a vision exam.  And, dental exams are required for grades Kindergarten, second and sixth.

Any student participating in a sport is required to have a sports physical prior to the beginning of their sport season.  This includes students in grades fifth and sixth, junior high, and high school.

Health examination forms are available at the school and may be picked up at any time or are available on the district web site.
